An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

An employee was loading material from a dock onto a barge. A tugboat struck the barge. The employee was knocked off a ladder, hit the side of the barge, and fell into the water. The employee suffered broken ribs and a lacerated liver and was hospitalized.

An employee was operating a combine to harvest soybeans when an access door became unlatched, exposing a rotating auger. As the employee attempted to re-latch the door, the rotating auger amputated the distal phalanx of the employee's third right finger.

On March 29, 2018, two employees were using a propane-powered forklift to treat seed in a seed warehouse when they became ill. They were both hospitalized with carbon monoxide poisoning from the forklift.

An employee was walking on a sidewalk and slipped on ice and fell. The employee was hospitalized with a head contusion.

PHOENIX MANUFACTURING, LLC

An employee was lying on a floor dolly on his back underneath a newly fabricated bin weighing approximately 1,000 lbs. The bin was supported on all four corners by 6-foot-long metal saw horses. The employee was painting the underside of the bin using a spray gun. When he attempted to scoot the dolly along the floor, his foot pushed against one of the saw horses, causing it to move. The saw horse pivoted out from under the bin, which then tipped and fell on the employee, breaking his arm.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

NYU Langone Hospital-Brooklyn

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.
