An employee was descending the stationary ladder of a feed bin when he missed a rung and fell 15 feet to the ground. The employee sustained fractures.

An employee was changing a bumper on a trailer truck when the bumper fell onto his left thumb tip and amputated it.

An employee was rolling down the side curtains of a flatbed trailer when the handle of the revolving cover began spinning rapidly, striking the employee on the thumb. The employee sustained an amputation to the tip of the left thumb.

An employee was handling the locking mechanism on a tractor trailer coupling safety pin. The rear locking device popped free and struck the employee's left hand, which was pinned against the upper supporting member. The employee's ring and little fingertips were crushed; the employee was hospitalized, and the fingertips were medically amputated.

On March 16, 2021, an employee slipped on black ice (concealed by recent snow) while stepping out of a building. The employee fell, suffering a broken left wrist.

An employee was using a crane. The hook slipped and a fitting fell onto a finger on the employee's left hand causing a fracture and laceration.

An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.

An employee was using a forklift to load pallets of product on a trailer. He exited the forklift to adjust a pallet by hand. The pallet fell on him, resulting in an injury to the left leg.

An employee was lifting a 500-pound steel counterweight off a pallet using a magnetic hold jib crane. The counterweight detached from the crane and the employee sustained a left foot/toe fracture that required surgery.

An employee was using a pipe wrench to turn a 42-foot-long steel pipe on jack stands to weld the bottom portion. The pipe fell off the stands, striking the injured employee on his left shin. The employee sustained a left leg fracture at the shin area as well as a tibia fracture that required surgery.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

An employee was hooking up a set of 28-foot trailers, attaching a converter dolly to the lead trailer's pintle hook. He backed up the lead trailer and parked it a foot from the converter dolly, then lifted the converter dolly and rolled it to the lead trailer to set it onto the hook. The converter dolly kicked up, and the employee's left hand was caught between it and the trailer. He suffered a severe laceration to the palm between the ring finger and middle finger.

An employee was cleaning the windshield of his truck. When he stepped down from the truck, he lost balance and reached to catch himself using the side mirror. The impact fractured his left wrist. The employee was hospitalized and required surgery.

An employee was standing on the DOT bumper on the back of a standard 53-foot trailer. The employee lost his footing and his grip on the handle bar, and fell approximately 42 inches to the ground. The employee was hospitalized with a fractured left hip/femur.

A driver was making a delivery. As he was climbing onto his trailer to get boxes, the strap he was using to pull himself up broke. He fell from the back of the trailer to the ground, landing on his right leg and breaking his femur.

An employee slipped while climbing a ladder on a skidder, fell to the ground, and suffered a tibia/fibula fracture. The employee was hospitalized.

An employee was lowering the bucket of a track loader that contained a grade stake and the grade stake punctured his foot, resulting in hospitalization and surgery.

An employee was using a pike to move a pallet of sugar that was stuck in the powder racking. The employee fell backward, landing on his right leg. The employee's leg was fractured and he was hospitalized.

An employee was working to fix the forks on a forklift when the fork(s) fell on her toes. The second and third toes on her right foot were fractured, lacerated, and required stitches.

An employee was delivering a 65-foot boom lift to a rental customer's construction site. The employee set up the ramp truck in the parking lot adjacent to the construction site. He elevated the boom 4-5 feet to clear a temporary fence. While driving the boom lift off the ramp, the wheels slid to the driver's side of the ramp truck, ultimately sliding off the ramp. The boom lift fell off the side of the truck and rolled onto its side. The employee sustained a fractured clavicle, a dislocated shoulder, separated/fractured ribs, a punctured lung, and a fractured left femur. The employee required surgery.
