An employee was walking between machines when he tripped on a bolt that was protruding up from the base of a piece of equipment and fell to the concrete floor. He sustained a compound fracture to his right femur that required surgery and a laceration to the upper lip area.
An employee was operating a hook and winch that was connected to a paint booth. The hook retracted and the employee's finger was caught between the hook and the outside winch framework, resulting in a fingertip amputation.
