An employee was walking through a storage space near the wash area when they slipped on a piece of debris and fell on the concrete floor. The employee's left femur was fractured.

An employee entered a poultry house to identify an issue with a disabled lift when he was struck by a forklift. The employee sustained three fractured ribs and a partially collapsed lung. He also sustained scratches on his head and swelling around his right eye.

An employee was using a hose to clean out the scale conveyor belt when he noticed a piece of product stuck in the conveyor. While removing it, the employee's right arm was caught in the running conveyor, resulting in a right arm fracture and hospitalization.

An employee stepped down onto a stairway with seven risers, putting both of his hands on the rails. He fell onto his arms and slid down the stairway while still holding the rails. His left leg made contact with the concrete floor first, and his tibia and fibula were broken. He was hospitalized, requiring surgery.

An employee was cleaning/foaming the floors between lines 2 and 3 when they slipped on a piece of chicken skin and twisted their leg. The employee was hospitalized with a fractured tibial plateau.

An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

The injured employee was putting away materials on the warehouse floor as a co-worker was pushing a pallet onto a nearby shelf. The pallet knocked another pallet forward, causing it to fall onto the injured employee's shoulders. The employee suffered fractures in their right and left knees and ankles.

Employees were moving a single man lift into a building and reclining the lift to position it to fit through the door. The lift shifted and fell, causing the employee to sustain fractures to the left tibia and fibula. The employee was hospitalized and had surgery.

An employee was moving a light tower so it could be hitched to a truck. He grabbed the tongue of the trailer hitch on the light tower to slide it to the left. The tongue jack fell off, causing the light tower tongue to drop on the employee's right hand. The employee's middle fingertip was amputated.

An employee was unloading a forklift from a trailer to the ground. The employee was climbing down the ladder of the trailer when their foot slipped through one of the rungs, causing them to fall backward onto the forklift forks. The employee was hospitalized for fractured ribs.

An employee was troubleshooting a thigh deboner on a poultry processing line. As he worked to dislodge a thigh bone caught in the blade mechanism, the machine amputated the first and second digits on his left hand. He was hospitalized.

On September 25, 2025, an employee was exiting a yard truck. They slipped and fell approximately 3 feet from the top tread of the steps to the concrete below. The employee sustained a fractured left hip.

On September 17, 2025, an employee was cleaning a cutting machine when the cutting blade amputated their right index finger below the first knuckle.

An employee was operating a stand-up forklift, picking product for an order. The forklift struck a metal rack and the employee's left arm was caught between the forklift and the rack. He was hospitalized with a fracture to the neck of his radius and an open fracture to the olecranon (elbow).

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
