On Friday, January 13, 2017, at approximately 8:20 AM, an employee was moving rebar on the third floor when he stepped out onto unsupported plywood and fell approximately 10 feet onto the second floor stairway. The guardrail had been removed the previous evening to allow removal of formwork and was not reinstalled. The employee suffered a compression fracture of the T11 vertebrae.
