An employee was manually moving a 55-gallon steel drum away from other drums on a pallet in order to pick up the drum using a drum grabbing machine. The drum shifted and pinched his left pinky above the knuckle. The employee sustained an amputation of the left pinky fingertip.

Material was being loaded in a metal building laminating machine. The machine's saw blade came off, cutting an employee's foot.

A driver was at a customer's location unloading fiberglass insulation on pallets. As the driver was removing a pallet, the top bags got caught on the door and shifted. The driver tried to shift them back and fell from the liftgate at the back of the truck. The driver sustained a cut on the leg and possible bone damage.

An employee was producing a roll of metal building insulation on the laminating machine. While trying to fix a problem with the machine, the employee's left hand was pulled between two rollers and causing a left forearm fracture.

TForce Freight

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

Liveo Research, Inc.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

During a material handling operation, an employee's fingertip was caught under an air conditioning unit as the unit was lowered onto a cart. The employee's fingertip was amputated.

An employee was removing a bolt from a tractor hub when their right index finger was caught between the hub and the frame. The employee's fingertip was crushed and the employee was hospitalized.

Employees were at the trucks to leave for lunch. The injured employee was putting the cone back after the crew leader pulled the truck forward from the designated worker parking area. The injured employee then ran back to the truck. As he went to get in the truck, he said the upper thigh area of his leg was hurting. He laid on the ground outside the truck and crew leader could see a bump/bulge in the the injured employee's upper thigh area below his hip. The employee sustained a fractured femur and required surgery.

An employee was helping a contractor unload a delivery of two new dock plates from a trailer onto a forklift. The employee s left index and middle fingers were pinched by the dock plates, resulting in amputations to both fingers above the first joint.

An employee was helping to load a customer's truck. The employee stepped toward a forklift to get paperwork from the driver. The employee's right lower leg and foot were run over by the back tire of the forklift. The employee was hospitalized with fractures to his lower right leg.

An employee was moving wet bundles of shingles on a truck bed, when one of the bundles slipped out of his hands. He tripped over a pallet, fell off the bed of the truck, and landed on his left side. The employee sustained fractures to three ribs, and a displaced iliac fracture.

On August 12, 2025, an employee was loading shingles onto the roof of a new-construction home when be began to suffer from heat exhaustion and dehydration.

An employee was walking in a garage toward her delivery vehicle. As she stepped down a curb near her vehicle, she fell and landed on the concrete garage floor, resulting in a hip injury that required hospitalization.

Loram Maintenance of Way, Inc.

Employees were performing maintenance operations on a railroad line when the machine experienced a load conveyor motor failure. While clearing the machine to allow for repairs, an employee pinned the left side of the swing conveyor. The operator on the right side then requested a slight bump to the left to align the right side for pinning. Once the swing was in position, the operator went to pin the right side and the chain tensioner failed, resulting in an abrupt release of the swing chain. The chain snapped back and struck the operator s right hand, resulting in a 1-inch laceration and a fractured fifth metacarpal. The employee was hospitalized and required surgery on the hand.

An employee was decommissioning a substation. The tower switch was being set down on the ground when the switch blade moved. The employee's finger was crushed and amputated.
