An employee was installing a security camera while on a combination ladder. The ladder tipped over and the employee fell about 6 feet, landing on his right arm. The forearm was broken and an artery was severed.

An employee was on an extension ladder approximately 4 feet off the ground opening a junction box on the exterior of a house to trace the wires for a security camera installation. Upon removal of the cover plate of the junction box, a swarm of wasps started to rapidly fly out of the junction box toward the employee. The employee was startled, lost balance on the ladder, and fell to the ground. The employee sustained a fractured left wrist and a fractured L1 vertebra, requiring hospitalization.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

M1 Support Services

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

FEDEX Ground Package System, Inc.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was working from an extension ladder, installing a security camera. The ladder fell out from under the employee, who fell to the ground 12 feet below and broke his right femur.

At about 9:20 a.m. on October 10, 2025, an employee was backing up a stand-up forklift in a storage warehouse to move pallets of goods from steel pallet storage racks. His right knee was pinned between the forklift and the steel racking, and his femur was broken near the knee. He was hospitalized, requiring surgery.

On 12/18/2024, a temporary employee was spotting for a forklift. The forklift was turning to put a pallet on a trailer for transport when the pallet struck the back of the employee's left leg. The employee sustained a broken bones in the lower left leg, ankle, and foot. He also sustained a laceration across the left foot.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
