An employee was using a conveyor belt to load a trailer. She went to place her foot on the working platform and fell approximately 2 feet from the platform between the conveyor and the trailer. The employee landed on the warehouse floor, sustaining injuries to the feet, shins, knees, and back.

An employee was pushing a flight out of the gate. While disconnecting the tow bar, the bar dropped onto the employee's left foot. The employee sustained a partial amputation of the second toe and a fractured big toe on the left foot.

An employee was driving a tug with a load into a warehouse. While the employee was trying to disconnect the dolly from the tow tractor hitch, the dolly brake tongue swung and struck his left knee. He suffered a broken leg and was hospitalized.

An employee was driving a double pallet jack down an aisle when another piece of equipment came through the aisle. Realizing that the two pieces of equipment were about to collide, the employee jumped off the pallet jack. It then ran over his leg, breaking his leg/ankle.

An employee was operating a forklift when the load shifted. The employee jumped from the forklift and it rolled backward, falling off the ramp and pinning the employee underneath it. They sustained head trauma and injuries to the shoulder, arm, pelvis, and leg.

An employee was operating a rider-style powered pallet jack. He lost his balance causing him to fall off and become trapped between it and a wall. He suffered a broken pelvis and was hospitalized.

An employee was operating a stand-on mower down a hill when the mower started sliding. The employee jumped off the mower and fell to the ground. Their hand was cut by the mower blade, resulting in a laceration and a thumb amputation.

An employee was driving a package car around a building on UPS property when the vehicle struck a bump. The force from hitting the bump unbuckled the employee's seatbelt and caused them to fall out of the vehicle onto the asphalt. The employee was hospitalized and required surgery for fractures to their hip and pelvis.

An employee was retrieving a water bottle from the operator compartment of a unit load device transporter. The transporter accelerated and ran over her foot, causing lacerations to the skin surrounding her toes that required hospitalization and surgery.

An employee had been accessing the cargo door to load an aircraft. They fell 15 feet from an aircraft belt loader to the tarmac. The employee was hospitalized with injuries to both arms.

An employee was standing on the back bumper of a package car as another employee was backing up the vehicle. Their toes were crushed between the dock leveler and the bumper of the vehicle. The employee sustained fractures to their foot and toes and required surgery.

An employee was tying down air pallets onto a dolly when a nearby forklift reversed and struck a transfer dolly, which then hit the dolly that the employee was working on. The air pallet dolly subsequently struck the employee and knocked him over, resulting in hospitalization for pain in their lower back, left hip, thigh, and leg.

An employee was opening a zip tie using a knife. The knife slipped and cut the employee's left index finger, resulting in a partial amputation (without bone loss) to the fingertip.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
