The employee was monitoring a plastic injection molding machine during it's start up/warm up phase. The employee observed excess product sitting in the bottom of the machine. As the employee was removing the product, a sprocket/gear on the machine grabbed the employee's left arm and pinched it just above the wrist. The employee sustained a cut on the left arm and wrist, which affected bones and tissue.

An employee was removing a fracturing pump suction/discharge manifold. The discharge manifold shifted, and his right index finger was pinched between it and the suction manifold. The top part of the finger was amputated.
