An employee was cleaning a pinch roller assembly when their right index finger got caught in the roller assembly, resulting in a partial de-gloving and amputation.

A an employee was cycling a brake press when it caught the tip of her right thumb, causing an amputation to the thumbtip.

An employee, who had gloves on, was using a powered drill tool to drill into an unsupported block of wood. The employee was holding the block of wood in his hands. When the employee actuated the drill, the glove got caught, resulting in a partial amputation of the thumb.

An employee was transferring propane from his bobtail truck to a storage tank. As the employee removed the hose, propane shot out and struck him in the mid-section, resulting in propane freeze burns from below the waist to the top of the knees. The employee was hospitalized.

On September 22, 2023, an employee was working on a chiller circuit in a mechanical yard. They sustained second-degree burns after being exposed to refrigerant.

An employee was tightening a fitting for monthly leak detection and repair when liquid natural gas was released, resulting in a cryogenic burn to the right hand.

On August 28, 2023, an employee was moving a CO2 tank when CO2 gas came out and froze the employee's left hand.

An employee was performing a pressure check and leak test on a unit when nitrogen gas was released from a valve core removal tool. The nitrogen gas was injected into the employee's right hand, resulting in a burn to the skin, swelling, and pain.

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was holding a piece of rubber while it was being cut on a band saw. The saw blade amputated his right middle fingertip without bone loss.

An employee was observing expansion rods on a coil expander going into a coil to ensure copper stubs weren't being bent. His left hand was caught between the locking block and locking block clamp on the power press machine, resulting in partial amputation of his left little finger from the distal interphalangeal joint.

An employee was setting up a press brake and rolling over a die when the tip of their left little finger was caught between the horizontal working surface of the press brake and die. The employee sustained partial amputation of their fingertip.

An employee was working to remove an access panel to begin performing maintenance on a machine. The set pins on the bottom of the panel were broken. The bottom kicked out and fell, striking the employee's left foot and resulting in a fracture to the big toe.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
