An employee was helping guide a pallet jack being moved by a crane. The pallet jack was lowered onto the employee's left index finger, causing a partial amputation (all of the fingernail and bone at the fingertip).

DPR Construction Inc.

On May 23, 2025, at approximately 10:50 a.m. the injured employee was assisting the operator of a mini excavator to ensure the grade/depth of an excavation. He crossed a trench that is 14" wide with a grade stick (2" x 2") in his hand and ended up on the ground behind the mini excavator. Simultaneously, the mini excavator moved backward and the base of the grading stick was caught by the tracks of the excavator, causing the employee to fall to the ground and become pinned underneath the excavator track. The employee sustained fractures to his right and left femurs, his pelvis, and his right ankle.

An employee was using a jointer machine to straighten a piece of wood. While trying to push the wood down, his hand slipped and contacted the rotating blade. The employee's left index, middle, and ring fingers were amputated at the first knuckle.

An employee was placing rigging hooks on a rebar mat to be lifted by a forklift. The rebar mat became stuck and when the employee went to free the mat, it suddenly came loose and pinched his left middle finger between two rebar mats, resulting in a crush injury and a laceration that required hospitalization.

An employee was standing next to scaffolding, waiting for a concrete pour. A wheelbarrow, being used to move bricks from one side of the scaffolds to the next, became unbalanced. It leaned sideways and the top layer of brick fell out of the wheelbarrow and off the scaffold, striking the employee about 45 feet below. The employee suffered a broken left humerus.

An employee was doing formwork when he moved and struck a horizontal rebar near his right eye. He suffered a laceration to his right eye that caused significant swelling, requiring hospitalization.

An employee was offloading containers from his truck using a deck that goes up and down. When the deck was up, he took a step and his foot went between the truck and the deck, resulting in a foot/ankle fracture.

An employee was moving 4-inch square tubing from a pallet to a four-post rack when his right ring finger was caught between the end of a tube and the rack. The employee's finger was partially amputated.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was installing insulation in a wall when she tripped on a hole (12" deep) and fell, hitting her head on the concrete floor. The employee sustained fractures to her leg and wrist, and lacerations to her head. The employee was hospitalized.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.

An employee was operating a power drill when it slipped. The drill bit punctured the employee's hand and they were hospitalized.
