On November 9, 2023, an employee was removing plywood from the west face of a basement level buck hoist to prepare to dismantle one of the cabs. When the employee pulled on the plywood, it fell and pulled a rope that partially amputated the employee's thumb.

An employee was holding a rigging line and assisting in the removal of an old light pole from its base. When the load was lifted by a forklift, it shifted. The tag line lifted the employee off the ground and propelled him through the air. The employee landed on the ground and sustained back fractures.

An employee was performing maintenance on a bucket truck when his right hand became caught in some rope, causing amputations to three fingers down to the first knuckle.

An employee was conducting a fall rescue demonstration. A 165-pound demonstration dummy was released from a height, putting tension on the lanyard. The employee's left hand, little finger, and thumb were caught in the wire rope lanyard, resulting in a medical amputation of the left little finger as well as lacerations to the left thumb.

An employee was using a crank and cable to lower a light tower due to windy conditions. The light shaft became hung up and released the tension in the cable, which wrapped around the employee's left hand, resulting in amputations of three fingers.

An employee was on a 20-foot ladder cleaning tile on the side of a building. The ladder slipped and he fell to the ground. He landed on his feet, resulting in hospitalization with fractures to both feet.

An employee was putting away stock in the aisles and began driving his order picker down the aisle with a pallet of items loaded on the forks. The pallet struck an upright of the warehouse racking and was pushed toward the employee, causing him to fall. The front edge of the pallet drove his right leg into the front panel of the order picker, fracturing his right leg above the ankle.

An employee was completing an inspection on a truck when the flywheel amputated their fingertip.

The injured employee was vacuuming out the trunk of a parked vehicle. Another employee was operating a vehicle when the brakes failed and the vehicle struck the back of the injured employee's legs pushing him into the vehicle he was vacuuming. The employee sustained a fractured leg.

An employee got out of his truck and closed the door when his right hand got caught in the door, resulting in a broken ring finger and an amputation of the middle fingertip that required hospitalization.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
