Final narrative
On 12/28/2020, while an employee was attempting to fix a pallet that was stuck to a palletizer, a photoelectric sensor on the palletizer tripped, causing a metal plate (apron) to retract. When the plate retracted, 11 cases of product dropped onto the employee's head, shoulder, and wrist, resulting in a small subarachnoid bleed and a sprain in their right wrist.
On December 15, 2022, at 5:40 AM, an employee was walking down a paved road to get to work from the parking lot. The employee slipped, fell, and struck his head, resulting in a subdural hematoma.
