An employee was moving a sausage linker when it started to tip over. The employee ran into another piece of equipment and was then hit by the linker. The employee suffered a hematoma and a blunt impact to the left leg and a cut on the little finger, and was hospitalized.

An employee was loading corrugated sheets into a box former from a 24" platform beside the equipment. As he was descending from the platform, he swung under the top rail to the floor and momentarily lost his balance. His right hand then entered the maintenance access opening on the backside of the box former, resulting in a partial digit amputation at the middle phalanx of the right index finger.

On September 4, 2020, an employee was performing sanitation duties on a horizontal auger when their left arm became caught in the auger. The employee sustained an amputation to the left arm above the elbow.

An employee was attempting to tack weld and repair a tubular steel livestock gate. The gate was in a bind due to a broken component and was positioned in such a way that a gap was present between the broken component and anchoring rail. As the employee placed the left hand on the anchoring rail, it dropped, pinching the soft tissue of the employee's left index finger between the component and anchor. As the employee pulled the hand back, the employee's left index finger was amputated (without bone loss).

An employee was using a T-handled tool to attempt to pull a food-filled drain basket out of the floor. She fell to her left side, landing in the hot water that was flooding the floor. She was burned on the left side, back, and forearm, and was hospitalized.

An employee was walking down the hallway when they tripped and fell to the floor, fracturing their right wrist.

An employee was walking down the hall to retrieve a vacuum when they stumbled and fell to the floor. The employee suffered wrist bruises and a kneecap fracture that required surgery.

An employee was carrying furniture backward during a delivery when he tripped and broke his right ankle.

An employee was installing a compressor piston. He tripped and fell backward while the piston was in his lap. As he landed, the little finger of his left hand became caught between the piston and compressor piping resulting in a partial amputation of the fingertip. The employee required stitches and the fingertip was reattached.

An employee was doing an oil change. He was walking to get his oil jug when he tripped on an open lift pad, and fell to the ground landing on his right side. The employee sustained a broken right femur and was hospitalized.

An employee tripped over a bin and fell to the floor. She landed on her hip and braced herself with her arm, resulting in a fractured hip and wrist.

An employee was reaching for a piece of angle iron when it fell onto his hand, which was on a table to support him as he reached for the overhead rack. He suffered a broken finger and was hospitalized, requiring surgery.

An employee was putting dough into a forming machine when their left little fingertip was amputated by the forming die.

An employee slipped and fell outside, landing on her hip on concrete. The hip was broken and she was hospitalized, requiring surgery.

An employee was working to open the liftgate on the back of a delivery truck. His left middle finger was pinched between the gate and its frame, resulting in a fingertip amputation.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
