An employee was trying to hang netting in the back of a chicken house. As the employee was securing the net to the corner of a ventilation fan guard, the guard broke loose, fell, and struck the employee's right hand. The employee suffered lacerations across and between the ring and little fingers.

UNICON OPERATING COMPANY, INC

At about 10:00 a.m. on March 15, 2021, an employee was loading a trailer with a forklift. As the forklift moved around the trailer, it became stuck in a hole. The employee was trying to free it when it broke loose and slid into trailer. The employee's left hand was smashed between trailer and the forklift's roll cage; the hand and fingers were broken.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

ABS Global, Inc.

The injured employee and a second employee were collecting genetic material from a bull inside the collection ring. The bull fell over onto the injured employee and they were hospitalized with a fractured left tibia.

An employee was cleaning a manure pit while in the elevated bucket of a bucket loader. While they were disconnecting a rope that holds the manure pump in place, the employee misstepped and fell 4 feet into the cement pit. The employee struck his head and sustained a forehead laceration and head trauma.

At 2:30 p.m. on October 13, 2021, an employee was on top of a pump truck, replacing the cone insert on the cyclone tank. He fell about 11 feet, striking a mounted toolbox before hitting the ground. He suffered a broken right leg below the knee, a compressed lower back vertebra, and a contusion to the right buttock. He was hospitalized.

An employee was attaching a powered industrial truck to a trailer in a parking lot. The employee slipped on ice and fell to the concrete, suffering a broken right arm.

An employee was training a young colt when the horse reared up and the saddle horn was pushed into the employee's abdomen rupturing the employee's spleen.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

The Goodyear Tire & Rubber Company

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

Big River Steel, LLC

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

American Tile & Marble Co., Inc

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
