Federal OSHA recorded a severe workplace injury at U.S. Department of Navy, 4477 McMahon Rd., Suite 115, INDIAN HEAD, MARYLAND 20640 on — Soreness, pain, hurt-nonspecified injury, affecting the back, including spine, spinal cord, unspecified.

While inspecting a portable dumpster, an employee climbed up the side of the dumpster using rungs that were integrated into the exterior frame of the dumpster. As he climbed the ladder, he fell backward approximately 2 feet to the ground, resulting in possible damage to his 5th and 6th vertebrae.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was making a fine adjustment after a tooling change on a pyrotechnic pellet press when the machine interface was actuated. The bottom piston on the press crushed his left hand, resulting in fractures to his index, middle, and ring fingers with tendon damage to the middle finger. The employee was hospitalized and required surgery.

An employee was testing a 40mm single-shot tactical launcher with the launcher mounted on her right shoulder and the barrel stabilized with her left hand. After the trigger was pulled, the 40mm barrel tore apart radially and she sustained injuries to her left extremity including the bicep, forearm, index finger, middle finger, and thumb.

An employee was loading parts into a press machine when the pneumatic/electrical 2" steel door closed on his hand, crushing it. The employee sustained the amputation of the whole hand and was hospitalized.

An employee was ascending a flight of stairs after exiting their vehicle in an adjacent employee parking lot. The employee stepped to the right to avoid another employee on the left who was descending the stairs. The employee lost their balance and fell on the stairs. They were hospitalized with a laceration/abrasion to their chin, a fractured left humerus, and bruising to their left knee.

A mail handler was moving BMCs on the loading dock when they fell from the loading dock to the concrete ground below. The employee was hospitalized with head trauma.

An employee was on an A-frame ladder while preforming demolition of a ceiling. A section of HVAC ductwork fell on the employee and knocked them to the ground approximately 12 feet below. The employee was hospitalized with a concussion and lacerations.

U.S. Department of Army

On the morning of July 24, 2025, an employee was conducting synthesis of a highly toxic experimental chemical (nerve agent) in a chemical fume hood. The operation was completed at approximately 11:30 AM. The employee was exposed to the chemical and began experiencing symptoms of illness/poisoning around 1:30 PM and was hospitalized.
