An employee was mowing a lawn when they suffered heat-related illness.

While inspecting the belts and pulleys on an air conditioner, the drive belt started and partially amputated an employee's finger.

An employee was using a 15-foot ladder to access a chill water line on a ceiling. The employee fell from the ladder to the concrete floor, suffering a broken left arm, left leg, and hip.

An employee lost consciousness and fell to the concrete pavement while blowing grass off of the concrete with a grass blower. The incident may be heat-related.

On October 26, 2017, at approximately 5:20 p.m., two employees were riding an RTV on the road. While turning left, the RTV was struck by a car. The injured employee was thrown from the passenger seat and struck his head on the ground, suffering a head injury and arterial bleeding in the brain. He was hospitalized and had surgery.

An employee was driving an aerial lift over a cable ramp. The employee backed up the aerial lift to cross over the cable ramp. During this process, the lift and basket bounced, causing the employee to bounce and land on his right foot, resulting in a fracture to his right leg.

An employee was operating a tractor in a potato field to pack down the soil. As the tractor tires drove over uneven soil, the tractor suspension moved abruptly, causing the enclosed operator seat compartment to also move. The employee was ejected from the seat and struck their head, arm and ribs inside the tractor compartment. The employee sustained a head and arm laceration as well as rib fractures.

An employee was operating an aerial lift when it impacted a berm on the ground. The employee lost balance and struck his abdomen against the guardrail system on the aerial lift, then fell backward and struck his head against the guardrail. The employee sustained a punctured spleen.

An employee was driving a skid steer when it struck a bump, causing the boom to come down and crush his feet.

An employee was operating a skid steer with dirt in the bucket. The skid steer drove over uneven ground, causing the bucket to tilt forward and hit the ground. As a result, the employee's head struck the bar on the front top part of the cab. The employee sustained a compression injury to the neck and upper back that included two injured vertebral discs that required surgery.

An employee was performing preventive maintenance on a chiller's glycol system. Pressurized glycol sprayed onto the employee's face; the employee ingested some of the glycol and developed nausea.

An employee was inside a shelving unit used to store food when a forklift went to move the shelving unit. The employee was struck by the forklift forks, resulting in a left ankle fracture.

An employee was lowering a dock plate into place to unload a trailer when their finger was caught and pinched between the dock plate and the trailer. They sustained a left middle finger laceration.

An employee was using an overhead crane to manipulate a part on a wash line. The part fell off the hook, then cut the employee's arm as the employee tried to catch it.

An employee was band cutting and sustained an amputation injury to her left index finger.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

A security employee was holstering her firearm when the gun discharged a bullet into her right leg. The employee was hospitalized.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.

An employee was walking on a sidewalk and stubbed his toe on an elevated portion of concrete, causing him to trip and fall. The employee's right knee was dislocated.
