An employee was using a machine to cut overlay for tires into smaller pieces. The employee was adjusting a strip of overlay when their left thumb was caught between the strip and a rotating bar. The thumb tip was amputated.

An employee was performing their normal duties as a tire assembler when a tire's sidewall came off the guide on a machine. The employee reached over the drum to re-engage the sidewall back onto the guide. While making the adjustment, the machine cycled and the sidewall knife contacted the employee's right middle finger amputating the fingertip between the bottom of the fingernail and the knuckle closest to the fingernail.

After lifting a tire onto a conveyor belt an employee experienced abdominal pain. The employee was hospitalized for a hernia.

An employee was trying to dislodge a material truck cart in an automated storage and retrieval machine. The employee dislodged the cart and was struck by the pusher arm, suffering a broken leg.

A fabric calendar operator's right thumb was caught between fabric and a roller. The tip of the thumb was amputated.

An employee was going to the breakroom when they tripped over a chair and fell, resulting in a hip fracture.

An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.

An employee tripped over a box and fell to the floor, fracturing their right arm.

An employee tripped over a piece of angle iron and fell to the floor sustaining a broken hip.

An employee was preparing for a CT scan when she tripped over a loose strap and fell to the floor, fracturing her hip.

An employee was replacing a tire on a wheelbarrow when the tire exploded. The employee sustained fractures to his right hand and an abrasion to the right side of his face. The employee was hospitalized and required surgery on his right hand.

An employee was guiding a customer's automobile onto the alignment rack when the vehicle drove over the rack and pinned the employee against the alignment computer. The employee sustained a bruised left hand and a fractured left fibula and was hospitalized.

An employee was working on a cushion ring (steel). As they lifted the cushion ring with both hands to flip it up, the ring slid down and the employee's right middle fingertip was crushed between the ring and the work table. The employee was hospitalized with a fracture, and their right middle fingertip was amputated.

An employee was lifting a tire when he felt pain in his left shoulder. He was hospitalized with a shoulder strain and left pneumothorax.

An employee was returning to their truck after collecting a check for a delivery of tires. As they were shutting the door on the back of their box truck they were struck by an electric all-terrain vehicle that was being serviced, resulting in a fractured left ankle, fractured elbow with bone through the skin, and fractured pelvis.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
