Talen Energy

Two employees were unplugging an ash line at the top of an ash transfer tank. The injured employee opened an inspection door near the top of the tank to see if ash was flowing. Hot ash came out of the inspection door while the employee was closing it. The hot fly ash contacted the employee's left hand and arm, resulting in first- and second-degree burns as well as some third-degree burns.

Electrical Contractors Inc.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

M1 Support Services

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

FEDEX Ground Package System, Inc.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

U.S. Postal Service - Los Angeles P&DC

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

Employees were pulling three-phase power lines to a power pole. An employee's hand was caught between a pulley and a rope, resulting in amputation of the employee's index, middle, and ring fingertips.

An employee was cutting through two secondary wires with a battery-powered cutting tool. The wires were energized at 120/240 volts; when the employee cut through the insulation around a wire, an arc flash burned the employee's left thumb.

An employee had been preparing material for a lineman at a jobsite and was pulling up wire on a hand line. The employee began to feel ill, suffering from heat illness.

An employee was fixing an issue with seed cleaning equipment when he slipped on the ladder he was on. He went to catch himself and his hand went into a moving belt and pulley on the cleaner. The employee sustained the amputation of his right middle and index fingertips just below the fingernails.

An employee was riding an all-terrain vehicle (ATV) to move a bison herd. He was riding up the side of a steep hill when the ATV struck a rock and rolled, ejecting the employee before rolling over him. The employee suffered broken vertebrae in his back that required hospitalization.
