An employee was loading a flatbed trailer when a forklift bumped into the trailer. A wheelset began to roll in his direction and the employee jumped off the trailer to avoid it. The wheelset caught him on the leg and he landed on his shoulder on the ground. He sustained a laceration below the left kneecap and a dislocated shoulder.

An employee jumped off the loading dock to assist a package delivery driver who fell. He sustained an ankle fracture that required surgery.

An employee was operating a punch press when he sustained injuries to his left index and middle fingers, resulting in the amputation of both fingertips.

The employee went to avoid a pallet in his path when the vehicle collided with a rack, causing the pallets to fall from the vehicle. His leg was pinned between the order picker and the stack of pallets, resulting in leg fractures.

A forklift operator fell off the forklift and the forklift ran over his leg. The employee was hospitalized with head and neck injuries.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
