An employee was feeding veneer into the dryer when the veneer became jammed. The employee's right arm was caught in the chain and sprocket while unjamming the veneer, resulting in amputation of the employee's right arm up to the elbow.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

Alsco Inc.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

Scotch Plywood Company, Inc.

An employee was working to dislodge a piece of stray veneer from a conveyor. The employee set up a ladder against the machine and climbed the ladder. He placed his left hand on the rail of the conveyor and his fingers were caught in the chain and sprocket assembly that powers the conveyor. Three fingers were amputated. The machine was guarded at the time.

Georgia Pacific Wood Products

A maintenance employee was changing a shaft on a piece of equipment that had been isolated for repair. While positioning the shaft into place, the shaft fell approximately two feet and pinched the employee's left index finger. The employee suffered a partial amputation of the fingertip without bone loss.

West Fraser

An employee was using a cart to move saw blades. The cart tipped over, striking and pinning the employee under the blades. The employee sustained severe lacerations to their torso and legs that required surgery.

An employee was labeling and writing on the back of a slab of quartz. The material was leaned toward him when the A-frame it was resting on shifted, causing the material to fall onto his right leg and break it.

An employee performed maintenance on various machines when they began to feel hot and unwell, and they experienced cramping. The employee was hospitalized with heat stress or dehydration.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.
