An employee was putting a conveyor belt back on a sprocket. The conveyor was activated and amputated the tip of the employee's right index finger (without bone loss).

Employees went to the trim saw control panel to investigate a malfunction. They ascended the baghouse ladder. A chemical explosion occurred within the baghouse. Two employees sustained burns to their face, neck, and arms.

JELD-WEN, Inc.

An employee was preparing a boiler for maintenance. The hatch doors to the boiler were being opened. After opening the left door, the employee began to open the right door; as the employee removed the bolts to open the hatch, the hatch dislodged from the top hanger supporting bracket, fell, and struck the employee's left side. The employee suffered fractures to the skull, collarbone, wrist, and hand, all on the left side.

An employee was transferring a load of bifold doors onto rollers. The employee's right ring fingertip was pinched between the rollers and the doors and the fingertip was amputated.

An employee was operating a chop saw to cut scrap material when his right hand contacted the saw blade. The employee sustained a laceration to the top of the hand between the middle and ring fingers.

An employee was removing groove filler from a vinyl extrusion piece using a handheld cutting tool. The tool lacerated the back of the employee's left hand and the employee was hospitalized.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

On October 14, 2025, an employee was cutting lumber on an upcut saw. When he activated the saw to cut a short piece, the blade struck the tip of his right thumb, resulting in a partial amputation.

An employee was using a table saw to cut strips of plywood. The employee's left hand came into contact with the sawblade, which amputated the index and middle fingers as well as lacerating the hand. The employee was hospitalized, requiring surgery.

An employee was loading four sliding glass door panels into a truck. He was turning to get a cargo strap when the panels fell onto him and broke his neck.

His hands began to cramp; he got out of his truck and collapsed on the road. He suffered an acute kidney injury and rhabdomyolysis caused by dehydration and heat illness.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.
