An employee was operating a forklift in a trailer to move product when the forklift wheel caught on the dock plate and became stuck. When the forklift cleared the edge of the plate the employee's leg was pinned between the forklift and the trailer wall, resulting in a broken leg above the ankle.

An employee fell 11 feet from an elevated order picker platform. The employee was wearing a body harness, but was not tied off prior to the fall. He was hospitalized and treated for two broken ribs, bruised lungs and facial lacerations.
