Final narrative
On March 18, 2021, at approximately at 3:30 PM, five employees were lifting a car transmission on a pallet off of a truck on a rainy day. While lifting the pallet, the transmission slid and the five employees quickly set the pallet on the truck again and the injured employee's finger was trapped between the truck and the pallet resulting in a fingertip amputation.
An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.
