An employee was moving a barge upriver, using a winch and cable to give slack in the mooring lines so they could be removed. As the cable reeled in on the winch, it caught the employee's life vest and pulled him into the winch pulley bolted to a stanchion. He suffered a broken hip and was hospitalized.

An employee had been removing phosphorus pentasulfide while on a scaffold. He was using a non-sparking tool to dislodge the phosphoric material. The employee noticed a flash of flame. He descended the scaffold, but the flash ignited residual phosphorus pentasulfide that was on the outside of the employee's personal protective equipment (PPE). He sustained thermal burns to his right hand and right calf.

An employee was squatting down to knock some rubber buildup loose on the inside of a diverter feed hose when the valve diverted in the direction of his left hand, pinching the tip of his left middle finger between the diverter feed hose and the diverter housing. The employee suffered a fingertip amputation.

A millwright was operating a rotating lathe when it caught his right hand. He suffered an open fracture to the little finger and a laceration to the index finger. He was hospitalized, requiring surgery.

Two employees were using a golf cart to transit from one production building to the other when the golf cart veered off the road and turned over into a ditch. The cart landed on one employee's right leg, breaking it in several places.

Employees were unloading logs on a logging vessel. The injured employee was standing on the deck as a load came in. The sling popped off the load and the end of the choker struck the employee's lower left leg, resulting in a shin fracture.

An employee was exiting a confined space while carrying a pressure gun when the trigger activated. Water penetrated the employee's left arm, resulting in injury.

The employee was putting shackles on the grab of a vessel-operated crane. As the crane was lifting the grab, it started to swing toward the hopper. The employee reached to stop it when they were caught and lifted by the grab and pinned between it and the hopper. The employee sustained a pelvic fracture and lacerations to the arm and pelvis.

An employee was launching a boat during rough sea conditions. The employee lost balance, causing their left index and middle fingertips to be caught in the eye of the bow line, resulting in amputation.

Two employees were in a boat pulling a 45-pound anchor out of the water by the rope. The rope tightened and as the employees were attempting to create slack in the rope, the injured employee began to slip. His hand was caught between the rope and the inside of the platform boat, resulting in amputation of the right middle finger with bone loss.

An employee was working to free a load by grabbing and shaking the live side of a tugger line. Once the load became free and progressed upward, the employee's hand was caught and pulled into a snatch block. The employee's right thumb was broken and lacerated. The employee was hospitalized and required a surgical amputation.

An employee was operating a drill rig when when their hand got caught between a 1/4-inch steel cable and a drill rod, resulting in the amputation of four fingers.

An employee was overseeing the operation of a rope being respooled onto the drum of a wire winding machine. He went to remove a piece of wooden reel from the rope. His glove got snagged and his right hand was pulled into the roller, pinching his fingers between the machine's roller and the rope. The middle and index fingers were crushed.

WEST AFFUM DEVELOPMENT CO LLC

An employee was visiting a patient's home to fit them with a wearable cardiac defibrillator. As he was walking from the front door to his car, a large dog broke free from a chain and attacked him. He was bitten on the forearm and fell. The patient was then able to restrain the dog. The employee was hospitalized with severe dog bites and required surgery.

An employee tripped over a junior I-beam about 4 feet long and 6 inches tall that was installed for circulating water valves and fell to the ground, resulting in a broken right-side rib.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.
