An employee was on a ladder, performing duct work in a ceiling. Exposed wiring shocked the employee's right ear/neck area and he fell off the ladder to the floor. He suffered a dislocation at the back of his right shoulder, and hematoma in the lower back.

An employee was helping another company's employees remove a trash cart from a dumpster when their finger was pinched by an opening on the trash cart, resulting in an avulsion and fracture to their left middle finger.

An employee was installing pipe hanger into the ceiling. The employee was working from a 12-foot step ladder using a full body harness with an MSA beam clamp and self-retracting lifeline attached to a 5-inch I-beam. The anchor point was located approximately 3 to 4 feet to the rear of the employee's position with the employee facing away from the anchor point. The beam clamp and self-retracting lifeline became detached from the 5-inch beam allowing the devices to swing downward towards the employee's position and striking the ladder's lower rung. The worker fell from the ladder landing feet first on the concrete floor below resulting in fractures to both lower extremities.

An employee's fingertip was amputated while aligning an 8-inch PVC drain pipe.

An employee was unloading a trailer when a box fell and struck the employee in the face. The employee suffered a concussion and was hospitalized.

An employee had been cutting lumber for a shed wall. A load of 16-foot trim pieces fell onto the employee causing vertebrae and ankle fractures.

An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.

An employee was in a sandblasting area, preparing the surface of a plate heat exchanger frame for painting. The frame fell on the employee, who suffered fractures to vertebrae, the neck, pelvis, and rib cage; and punctured lungs.

An employee was installing doors when one of the doors fell on him, fracturing his lower leg. The employee was hospitalized.

An employee was on a ladder four feet above the ground to install insulation. The ladder began to slide and the employee jumped to the shop floor, breaking his left tibia and fibula at the ankle. He was hospitalized, requiring surgery.

An employee was replacing a rubber pad on the cylinder of a test station. Their right index finger was caught between a hydrant and a pneumatic cylinder of the test machine resulting in amputation of the fingertip.

An employee was driving a pallet jack. As they went to turn down an aisle, their foot was pinned against the pallet racking, resulting in a foot injury and laceration.

An employee was operating a hydraulic press, loading pre-cut tubes of copper into a feeder when the die closed on their right hand. The employee's right second, third, and forth fingers were amputated at the second knuckle.

An employee was conducting grinding operations on a cast iron pipe using a portable grinder. The grinder kicked back and struck the employee's right wrist and knee. The employee sustained a laceration to his right knee and a cut to his right wrist that injured an artery and a tendon.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

A security employee was holstering her firearm when the gun discharged a bullet into her right leg. The employee was hospitalized.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.

An employee was walking on a sidewalk and stubbed his toe on an elevated portion of concrete, causing him to trip and fall. The employee's right knee was dislocated.
