An employee was walking through a storage space near the wash area when they slipped on a piece of debris and fell on the concrete floor. The employee's left femur was fractured.

An employee entered a poultry house to identify an issue with a disabled lift when he was struck by a forklift. The employee sustained three fractured ribs and a partially collapsed lung. He also sustained scratches on his head and swelling around his right eye.

An employee was using a hose to clean out the scale conveyor belt when he noticed a piece of product stuck in the conveyor. While removing it, the employee's right arm was caught in the running conveyor, resulting in a right arm fracture and hospitalization.

An employee stepped down onto a stairway with seven risers, putting both of his hands on the rails. He fell onto his arms and slid down the stairway while still holding the rails. His left leg made contact with the concrete floor first, and his tibia and fibula were broken. He was hospitalized, requiring surgery.

An employee was cleaning/foaming the floors between lines 2 and 3 when they slipped on a piece of chicken skin and twisted their leg. The employee was hospitalized with a fractured tibial plateau.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was unloading a forklift from a trailer to the ground. The employee was climbing down the ladder of the trailer when their foot slipped through one of the rungs, causing them to fall backward onto the forklift forks. The employee was hospitalized for fractured ribs.

An employee was troubleshooting a thigh deboner on a poultry processing line. As he worked to dislodge a thigh bone caught in the blade mechanism, the machine amputated the first and second digits on his left hand. He was hospitalized.

On September 25, 2025, an employee was exiting a yard truck. They slipped and fell approximately 3 feet from the top tread of the steps to the concrete below. The employee sustained a fractured left hip.

On September 17, 2025, an employee was cleaning a cutting machine when the cutting blade amputated their right index finger below the first knuckle.

An employee was operating a stand-up forklift, picking product for an order. The forklift struck a metal rack and the employee's left arm was caught between the forklift and the rack. He was hospitalized with a fracture to the neck of his radius and an open fracture to the olecranon (elbow).

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was shot in the leg with a pistol during an altercation with two people who had entered the store without permission.
