An employee was attempting to feed film through a jammed sealing machine when it closed on and lacerated his left middle fingertip. His fingertip had to be medically amputated. The machine was guarded and not locked out at the time of the incident.

A store employee was pushed by a shoplifter into the frame of the entrance door to the store. The employee's head struck the doorframe resulting in head injuries and contusions on her hand and arm. The employee was hospitalized.

The employee sustained lacerations to the head and face, a fractured forearm, and a fractured little finger.

A teacher's assistant was attending to a student in the quiet room when they were pushed by another student. The employee fell to the floor and sustained a left hip fracture.

An employee was using a pneumatic nail gun to assemble crates. A nail punctured the employee's left index finger.

On December 10, 2024, an employee was using a table saw to cut wood and amputated their fingertip. The saw was guarded at the time.

An employee was driving a counterbalance forklift. When he approached the opening of a demising wall, his lower left leg/foot got caught between the forklift and the bollard. He sustained a fracture.

An employee was cutting boards using a table saw when their right hand came in contact with the blade, resulting in amputations to their middle, ring, and little fingers.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
