While waiting to assist with delivering a transformer from a flatbed truck, an employee went to get a drink of water. He returned to the work area and set his water bottle down. He later went to take a drink and picked up a different bottle that contained denatured alcohol. The employee was hospitalized due to poisoning.

An employee was carrying a bottle of door seal solution containing sodium silicate. The employee bumped their arm, causing them to ingest the solution. The employee suffered from wheezing, nausea, and breathing difficulties.

An employee was preparing a job area for an air conditioning replacement when they ingested drain solvent.

An employee was siphoning water from a tank and ingested chemicals that were in the siphon hose. He was hospitalized.

An employee was siphoning degreaser from a 55-gallon drum. Some of the caustic solution entered his mouth and throat, causing burns. He was hospitalized.

An employee was helping unload steel slit coils on a slitting line, using an overhead crane with a C-hook. The coil did not fit properly onto a turnstile. Another operator went to remove the coil from the C-hook using a forklift while the employee held the C-hook steady. The forklift contacted the coil, causing it to fall from the hook. It landed on the employee s foot, breaking four bones. The incident took place during a training activity.

An employee was trying to open the stuck tailgate of a truck. When he pulled on it, it gave way, causing him to fall back and hit his head on the pavement. He suffered a head laceration/contusion.

An employee was mounting an ATV tire. As he was inflating it, the rim came out of the tire and struck his right leg, breaking it below the knee.

On July 25, 2025, an employee was lowering a vehicle on a lift. The employee's right index finger was caught in the lift and partially amputated.

Two employees were setting up a mandrel in the spindle of a machine. The mandrel was side-shifted while an employee's hand was on the mast of a powered industrial truck. The employee suffered crushing injuries that resulted in amputations to the middle and ring fingertips.

An employee had been loading a double deck press. The employee saw a mold had been sent in with the C-hook still attached on the belly bar. As he went to remove the hook from the mold, his right thumb got caught on the C-hook. The employee's thumb was partially amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.
