An employee was operating a stand-up forklift to move metal cages that contained finished rubber tires. The employee had just moved a cage and did not have a load on the forks when the forklift struck a metal column. The employee was ejected from the forklift and her left foot was caught between the vehicle and a metal column, resulting in an open fracture to the lower left leg.

An employee was driving down an aisle when their powered industrial truck (PIT) drifted toward a column. The employee's left foot was caught between the PIT and column and fractured.

At about 5:45 p.m. on May 19, 2022, an employee was developed heat stress while cutting rubber blocks with a pneumatic press and manually stacking them onto separate pallets. The employee was hospitalized.

On March 20, 2022, an employee was driving a forklift in reverse when his left leg was caught between the forklift and a steel box. The leg was broken.

A stand-up powered industrial truck struck another powered industrial truck while turning. Its operator was ejected and was caught between the two pieces of equipment. The operator suffered a bone contusion and a small pelvic fracture at the left side of the hip.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

An employee was operating a rider pallet jack when it struck a stand-up forklift. The forks struck both of the employee's ankles, resulting in hospitalization.

An employee was working to repair a machine with a broken metal stopper. She removed the stopper and used a roll of tape to temporarily secure it. While she was moving it back into place to fix the machine, the residual pressure of the plastic pallets forced the stopper down, resulting in her right thumb becoming stuck between two metal parts. The thumb was amputated.

An employee was changing the battery on his forklift. He removed the forklift's battery plate and exited the battery machine alignment track. The shift lead then went to retrieve a new battery. The employee entered the battery machine alignment track again to unplug the charging cable. The battery changing equipment proceeded forward and pinned the employee against the forklift. The employee was hospitalized with two fractured ribs.

An employee took a picture of the seal and license plate on a delivery truck after loading. The truck then reversed back to the dock and pinned the injured employee against the dock. The employee sustained an abdominal fracture.

An employee was using a hook tool to pull a pallet onto the lift gate of a tractor trailer. The hook detached from the underside of the pallet, causing the employee to fall backward off the lift gate. The employee landed on the concrete parking lot about 5 feet below, suffering fractures to the skull and two thoracic vertebrae.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
