An employee was removing a handrail system when they stepped back into a hole that contained hot liquid. The employee was hospitalized with second-degree burns to the right lower leg and spots on the right upper leg.

An employee began shaking and trembling while installing piping insulation on the third level of a structure. He was hospitalized, suffering from heat exhaustion and dehydration.

An employee was operating a forklift with a hopper attachment to dump trash in a dumpster. While releasing the handle to allow dumping, the employee placed his left hand between the hopper and the forklift tine to reach up to release the lever. When it released, the weight shifted, pinching his left ring finger between the hopper and the forklift tine. This resulted in a crushing injury that required amputation to left ring fingertip.

Griffith Steel Erection, Inc

An employee was unhooking a rebar curtain from a crane while at the top of a 28-foot steel wall that they were tied off to when the wall disconnected and folded. The employee fell and struck additional braces and material on the way down, resulting in a fractured jaw and missing teeth.

An employee was ascending a tree using climbing equipment. A large hollow space in the tree collapsed causing the employee to fall 30 feet to the ground. The employee was hospitalized for a broken right shoulder and broken ribs.

An employee was on a scissor lift at 30 feet when the lift hydraulics failed and the lift suddenly went straight down. As a result, the employee broke both ankles.

At 7:27 AM, two maintenance workers were working in an aerial lift that was 27 feet off of ground. They were servicing an overhead crane. Another overhead crane ran into the aerial lift, tipping it over. The two maintenance workers were hospitalized with fractures to their backs and other injuries.

An employee was laying bricks from the working platform of a scaffold. Boards gave way and the employee fell 25-30 feet to a dirt floor. The employee suffered a broken heel and a hairline fracture of the hand.

A crew was relocating a dumpster (containing approximately 200 pounds of demolished wire) to access the uninterruptible power supply (UPS) units they were servicing. During movement of the dumpster, a section of carpet tile dislodged from the floor, causing an employee to lose his footing, slip, and fall. He landed on his right wrist, resulting in a fracture.

An employee was cutting a board using a circular saw. The saw pinched, kicked back, and cut the employee's left ring and little fingers. The employee suffered an amputation to the little finger.

Severo Ortiz Construction LLC

An employee was using an aluminum extension ladder to remove gutters at a height of approximately 20 feet. As they were removing the gutters, part of the gutter collapsed and struck the ladder, causing it to move sideways. The employee fell from the ladder and sustained fractures to ribs on the left side and their right forearm.

An employee was using an electric saw to cut a piece of wood for a fence. The saw slipped out of his hand and lacerated his left leg. He was hospitalized.

An employee was erecting a scaffold. A side bracket was not secure. The employee stepped on the bracket to test it and fell 18 feet to the concrete floor resulting in fractures to the ankles and femurs. He was not using fall protection equipment.

T G Meat Center LLC

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
