An employee was transferring materials using a dolly cart. As he was trying to load the cart on a weighing scale, it tipped over on top of him, lacerating his nose and lip and breaking his ankle.

An employee was loading an order of office chairs onto the second level of a semi-trailer truck, which required other employees to pass the chairs up to him so he could roll them back across plywood flooring that he was sitting on. He then attempted to stand up on the edge of the sheet of plywood when the plywood shifted and tilted vertically, causing him to fall approximately 4.5 feet to the wooden floor of the trailer. He landed on his back, head, and neck. His head struck the floor, bounced up, and struck the floor again as it came down. He sustained back, neck, and other bodily injuries, requiring hospitalization.

An employee was operating a machine when a wire pulled his right glove resulting in amputation of the index and middle fingertips.

An employee was grinding chicken when their right index fingertip was caught in the grinder and amputated.

An employee was checking on a machine outside of the building and removed a cover to clean out a blocked area. His hand was caught by a rotary valve in a dust collection machine. The employee's fingertip was amputated.

A shear operator's hand and fingers were caught in a machine's point of operation. The operator's right index finger was crushed and was partially medically amputated.

An employee was operating a forklift. He parked on a ramp and got off the forklift to retrieve material, at which point the forklift rolled backward and ran over his leg. The leg was broken and he was hospitalized, requiring surgery.
