A temporary employee was standing on a conveyor when an alarm went off, signaling that a shredder was about to start up. The employee, startled, rolled off the conveyor and fell to the ground about 8-10 feet below. He suffered a broken pelvis and was hospitalized.
A material handler operator was moving recycling material. When the operator let go of the material, the grapple swung back in the other direction.
