An employee was ascending a flight of stairs after exiting their vehicle in an adjacent employee parking lot. The employee stepped to the right to avoid another employee on the left who was descending the stairs. The employee lost their balance and fell on the stairs. They were hospitalized with a laceration/abrasion to their chin, a fractured left humerus, and bruising to their left knee.

The injured employee was assisting a co-worker in moving a wheeled cabinet through a door jamb. One of the cabinet's wheels malfunctioned and the cabinet tipped in towards the door frame. The injured employee was caught between the door frame and the cabinet resulting in a fractured right femur.

An employee was working next to an engraving machine when it caught fire. While he tried to extinguish the fire with a portable extinguisher, he was overcome by the smoke.

An officer was participating in training when a flash-bang grenade exploded in or around his hand lacerating it.

On March 3, 2018, an employee was going to his vehicle when he was struck in the eye by shattered plexiglass light globe debris during a wind storm. He suffered face and eye lacerations, requiring hospitalization.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was patrolling on an all-terrain vehicle (ATV) when an accident occurred resulting in injuries to their back, neck, ribs, and nose, as well as road rash.

A trainee became ill due to heat and dehydration after completing the basic academy's outdoor fitness test that includes a 220 yard sprint, maximum number of push-ups, and a 1.5 mile run. He was hospitalized for kidney failure with nausea and lethargy.

Two employees were riding in a boat in a river when the boat struck an I-beam that was submerged in the water. The employees sustained injuries.

Two law enforcement officers were executing an arrest warrant. After entering the residence, the assailant opened fire with a pistol, shooting one deputy in the right ankle and the other in the left femur bone. Both deputies required hospitalization and surgery.

An employee was hiking within the area of responsibility around a station when they began to experience symptoms of a heat-related illness due to dehydration.

A mail handler was moving BMCs on the loading dock when they fell from the loading dock to the concrete ground below. The employee was hospitalized with head trauma.

An employee was on an A-frame ladder while preforming demolition of a ceiling. A section of HVAC ductwork fell on the employee and knocked them to the ground approximately 12 feet below. The employee was hospitalized with a concussion and lacerations.

On the morning of July 24, 2025, an employee was conducting synthesis of a highly toxic experimental chemical (nerve agent) in a chemical fume hood. The operation was completed at approximately 11:30 AM. The employee was exposed to the chemical and began experiencing symptoms of illness/poisoning around 1:30 PM and was hospitalized.
