An employee was winching cages down to a drop-deck live-haul trailer. The terminal tractor moved in reverse and the employee was pinned between the fifth wheel and the trailer kingpin area. The employee suffered multiple fractures and lacerations.

An employee opened the hatch of a meat processing machine for maintenance to remove product from underneath the machine. The hatch bumped and smashed the employee's left ring fingertip between the machine and the hatch, resulting in an amputation to the tip of the nail.
An employee was using a powered pallet jack to move product. The stabilizer/drive wheel lost contact with the floor. When the wheel caught traction, the employee's lower right leg was pinned between the jack and the wall. The employee suffered a compound fracture to the lower leg.
