An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was talking to a child on the playground, when the child grabbed her hand to lead her away. The employee stepped, lost her balance, and fell over. The employee was hospitalized with a fracture of her lower left leg and required surgery.

An employee was seated in the living room of a patient's residence, waiting for the patient to arrive home from school. The school bus driver honked the horn, signaling that the patient was ready to be assisted off the bus. The employee stood up and then tripped and fell over a rug and possibly the edge of an ottoman. The employee was hospitalized with a fractured right humerus at the shoulder.

An employee was assisting a client when the client fell on top of the employee. The employee's hip was fractured.

An employee was retrieving their purse and keys when they tripped over a cord and fell to the ground, resulting in a broken arm and five broken ribs.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
