On September 21, 2021, at approximately 2:30 a.m. an employee was cleaning a pastillator machine, a rotoform granulation system, when her right hand was caught and crushed in a running nip point. The employee sustained fractures and second and third degree burns to the right hand. The machine was not locked out or guarded at the time.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

An employee had been working to fill lump charcoal into 40-pound bags when the crusher jammed. He was standing at the filling station and went to clear the machine. The employee's gloved right hand was pulled into a belt and pulley, causing the amputation of his little finger and a severe laceration to his ring finger. The employee was hospitalized and required surgery. The machine was guarded and locked out/tagged out at the time.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
