An employee was raising a 65-pound plastic tank out of the dunk tank after the plastic tank failed a leak detection test. The plastic tank got caught on the arm of the dunk tank. The plastic tank then came loose from the dunk tank arm and fell, crushing the injured employee's right hand against the edge of the dunk tank. Their ring finger was surgically amputated at the distal interphalangeal joint.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was operating an electric stand-behind double pallet jack. He was turning around on the dry dock to go in the opposite direction toward the freezer. Midway through the turn, his left leg was caught between the pallet jack and some racking. The employee's left ankle was fractured.

An employee was reversing a transport truck out of a parking space when the side mirror of the truck contacted another tractor and it folded inward. The employee went to fold the mirror back out and their left ring finger slipped into a recessed area of the adjusting mechanism, causing the mirror base to amputate his left ring finger.

An employee was operating a stand-up reach truck. He was taking a small pallet of product through a dock area when another stand-up reach truck struck him. The steel forks punctured his lower right calf, also breaking a bone in the same area. He was hospitalized, requiring surgery.

On October 8, 2024, an employee was wiping down the core cutter machine when his cleaning rag was caught by the running belt pulling his right ring finger into the belt assembly and resulting in a partial amputation.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

A security employee was holstering her firearm when the gun discharged a bullet into her right leg. The employee was hospitalized.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.

An employee was walking on a sidewalk and stubbed his toe on an elevated portion of concrete, causing him to trip and fall. The employee's right knee was dislocated.
