An employee was operating a heavy-duty tractor truck on the upper ramp of a vessel deck. When the truck was halfway up the ramp, it started to roll down the ramp and went in the air on its rear tires before coming down. The employee suffered a pelvic fracture and dislocated hip.

An employee exited a vehicle in the Tyco maintenance area and was struck by a Tyco mechanic driving a second vehicle in the area.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

An employee was standing on the outside edge of a hopper barge while watching a skid steer push ore for an excavator (with a clamshell bucket) to unload. The excavator was on a work barge. The bucket of the excavator struck the employee and caused a near-amputation of their right arm and fractures of the ulna and radius of the left arm. The right arm required surgery.
