An employee was delivering a couch to a customer's house. The employee was lifting the couch over a living room banister when the couch began to fall. As the employee reached over the banister to catch the couch his left hand was caught on the couch, resulting in a wrist fracture.

An employee was changing the battery on a forklift when her left middle and ring fingers were caught between the battery and the structure. The middle fingertip was amputated and the ring finger sustained a laceration.
An employee was adjusting a dock plate in order to unload a truck. The employee's finger was pinched between the dock plate and the truck resulting in a partial amputation.
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
