An employee was marking a malfunctioning mold when their finger got caught between one section of rollers on the mold and the guarding on the next section of rollers. The employee's finger was amputated below the nailbed.

An employee was removing plastic wrap from a machine with the machine propped open, when the knife descended and cut their right thumb tip and index finger. The employee was hospitalized and required surgery and a medical amputation to the thumb tip and index finger.

An employee was removing a popsicle stick from a frozen treat machine when their glove was caught in the machine, resulting in a fingertip amputation on the right hand.

The employee was operating the wrapper knife machine which cuts plastic wrap that is used to package ice pops. The plastic wrap jammed underneath the guard. When the employee attempted to clear the jam, his left thumb was severely lacerated requiring surgery and hospitalization.

On November 3, 2016, at approximately 1:40 p.m., a temporary employee was cleaning production equipment with sodium hydroxide material in a 5-gallon bucket. The employee wore rain suit pants, gloves, goggles, and a long-sleeve shirt. The caustic material made contact with the employee's right forearm, resulting in a burn that required hospitalization and surgery.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

An employee was using an end mill to mill out a section of a metal door frame that he was fabricating. Metal shavings built up and he could not see the marking to cut, so he used a small brush to remove the shavings. While brushing off the shavings, his glove became caught in the spinning mill bit and pulled his hand in. The employee's right index finger contacted the spinning bit, resulting in a partial amputation to the side of the finger.

An employee was unjamming cookie dough from a machine when the rotating pump caught her left index and ring fingers, resulting in fingertip amputations.

An employee was climbing down a ladder when it slipped. The employee fell, suffering a broken scapula and broken ribs. The employee was hospitalized.

An employee was climbing down a 15-foot multipurpose ladder. The employee fell about 8 feet, landing on the concrete floor and the ladder itself. The employee suffered several injuries, all on the right side: broken ribs, a broken sternum, a broken temporal bone, broken shoulder, multiple fractures to the cheekbone, and a brain bleed.

An employee was rolling up the landing gear on his trailer when the handle began to unwind and struck his face, fracturing his jaw. The employee was hospitalized and required surgery.
