An employee was using a power washer to perform sanitation duties when the power washer wand sprayed and lacerated three toes. The employee was hospitalized.

An employee was standing on a bag of bentonite clay composite when it ignited and the employee's pants leg caught on fire. The employee then went to remove a gasoline can from a dual-axle trailer and sustained burns to his arms, right hand, and the right side of his abdomen.

An employee was burning a brush pile. They poured gasoline on the fire, resulting in burns to his face, arms, and hands.

An employee was helping to put out a fire in a wok when hot cooking oil splashed onto the employee. They sustained second-degree burns.

Two employees were working on an empty above-ground 2,500-barrel crude oil tank. Employee 1 was welding while employee 2 was watching. Employee 1 saw fire behind the welding smoke and notified employee 2. Before they could get off the tank, they both fell in the tank and sustained burns from the fire.

An employee was pressing pyrotechnic composition into pellets using a pressure tablet press. Upon pouring the composition into the small hopper, the composition ignited and produced a flash fire. The employee sustained bilateral burns to the hands and face.

While attending the slab line, an employee observed that dough was accumulating on the plastic blades. He proceeded to clean the blades. His left index finger came into contact with an adjacent blade that moves laterally (left to right), resulting in a partial amputation.

A sanitation employee entered the chemical room and slipped on the wet floor. When he slipped, he kicked over a bucket containing cleaning chemicals. The chemicals splashed onto the employee's clothing and burned his skin.

An employee's right middle fingertip was caught between a chain and a sprocket in a machine. The fingertip was amputated. The machine was unguarded at the time.

An employee had just finished unloading material from a truck. He was descending using the truck's built-in ladder when he missed the last rung, fell to the ground, and broke his hip and left wrist.

An employee was using a handheld grinder to cut a bolt. The grinding wheel broke apart and a fragment struck the employee s right hand, causing a laceration to the palm near the little finger.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
