At about 10:10 a.m. on November 4, 2021, an employee was positioning a jackhammer to load into a skid steer bucket. He was bent over at the waist, sliding the jackhammer to get it closer to the bucket, when the sole of his work boot was caught between the jackhammer and the lip of the bucket. He fell forward and his left hip struck the concrete floor, resulting in a fractured hip.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
