An employee was moving freight to the back of a trailer for the forklift operator to unload in the parking lot when they lost their footing and fell 44 inches from the back of the trailer to the ground, resulting in a contusion to the back of their head and a subdural sinus fracture.

On February 7, 2024, a forklift struck an employee as it backed up. The employee suffered a broken leg.

An employee was making a delivery at a customer's business. When the employee pulled the pallet jack over the lip of the door frame, he slipped but regained his balance. The employee heard a pop and then fell to the ground. The employee sustained injury to their quadriceps tendon and patellar tendon.

An employee was attempting to hook a converter dolly to a tractor when the dolly started moving. The employee tried to steer the dolly toward the pintle hook, and their thumb got caught between dolly and pintle hook. The employee sustained a partial amputation to their left thumb.

An employee was unloading freight from a trailer with a pallet jack. He fell out of the trailer, landed on the ground, and injured his right ankle.

On October 26, 2023, an employee was riding on the forks of a forklift that was moving a stack of glass panels. The forklift made a turn and the glass fell on the injured employee, causing lacerations and crush injuries to their hands.

An employee was on a tractor and moving a stack of lumber when a board shot out from the stack and struck the employee's right leg. Their tibia was fractured and required surgery.

An employee was using a powered industrial vehicle (PIV) equipped with specialized horizontal clamps to move multiple heavy paper rolls. The top roll came loose from the clamps, falling towards the cab of the lift and lacerating the employee's middle and ring fingers.

An employee was helping a forklift operator move a steel joist. The joist struck a forklift tire, and as the forklift started to move, the joist shifted and struck the employee's right leg, resulting in a fracture.

An employee was using a pallet jack to move product from a truck to a customer location. A box on the top of the pallet shifted while the pallet was going over the bump of a dock plate and fell on the employee s head. Afterward, the employee experienced numbness in their limbs.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

An employee was hooking up a set of 28-foot trailers, attaching a converter dolly to the lead trailer's pintle hook. He backed up the lead trailer and parked it a foot from the converter dolly, then lifted the converter dolly and rolled it to the lead trailer to set it onto the hook. The converter dolly kicked up, and the employee's left hand was caught between it and the trailer. He suffered a severe laceration to the palm between the ring finger and middle finger.

An employee was cleaning the windshield of his truck. When he stepped down from the truck, he lost balance and reached to catch himself using the side mirror. The impact fractured his left wrist. The employee was hospitalized and required surgery.

An employee was standing on the DOT bumper on the back of a standard 53-foot trailer. The employee lost his footing and his grip on the handle bar, and fell approximately 42 inches to the ground. The employee was hospitalized with a fractured left hip/femur.

A driver was making a delivery. As he was climbing onto his trailer to get boxes, the strap he was using to pull himself up broke. He fell from the back of the trailer to the ground, landing on his right leg and breaking his femur.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
