An employee was repairing polycarbonate roof panels on a cotton warehouse. The employee stepped on one of the polycarbonate roof panels and fell 21 feet to the concrete floor below. The employee sustained two broken hips, a broken nose, and broken ribs. The employee was hospitalized.
An employee climbed up an extension ladder and began using bungee cords to secure a tarp to a railcar. He hooked one end of a cord and then pulled the cord so it would reach the anchor point for the other end. The cord broke causing him to fall backward off the ladder approximately 10 to 12 feet to the ground. The employee sustained fractures to the right knee and left ankle, and his left heel was shattered.
