Four employees were on a warehouse roof replacing the metal sheets. An employee stepped on a piece of insulation that was not covered by the roof and fell about 30 feet to the concrete floor. The employee suffered cervical fractures and head trauma and was hospitalized.

An employee was performing grounds maintenance work. The employee stepped on the edge of a ventilation grate when it flipped over, causing the employee to fall about 30 feet to the ground below. They sustained two heel fractures and a left tibia fracture.
