On December 17, 2021, an employee was troubleshooting an issue with the conveyor line. The employee was trying to remove a dough ball from the conveyor between a proofing area and a dough press when she was caught by and pulled into a set of rollers. The employee was pulled in up to her shoulder area and pinched between the rollers. The employee suffered broken ribs, a punctured lung, nerve damage and had the bicep of her left arm torn from the bone. The employee was hospitalized.

An employee was working on a divider that makes balls of dough from a larger batch in an elevated hopper. The employee was removing dough that was leaking from the elevated hopper, when their hand was caught in the machine, resulting in an amputation to the tip of their right thumb.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

Flowers Baking Company of San Antonio LLC

A team was disassembling a dough pump s auger to allow the wastewater to drain from an allergen changeover. The auger became stuck due to suction caused by residual wet dough. A paddle was being used to pry the auger loose when the suction force suddenly released, causing the auger to retract unexpectedly. A finger on the employee's left hand was caught between the auger and the housing, leading to an amputation to that finger.

On February 4, 2025, at approximately 6:20 PM, an employee was cleaning the dough inside a divider when they sustained an amputation to their right index fingertip.

An employee was troubleshooting a mixer on the biscuit line when their hand became caught under the machine belt, resulting in fractures and an amputation of their middle fingertip. The machine was not locked out/tagged out at the time.

An employee was attempting to stop tools from falling into product when their right hand was lacerated by the blades in the dough chunker machine.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

General Thermodynamics, Inc.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
