An employee tripped over a box and fell while sweeping the floor, fracturing her left femur.

An employee was tying down a tractor on a flatbed trailer with tie down chains. He then stepped back off the trailer to the ground, and felt pain in his left hip. The employee sustained a torn hip flexor.

After rewiring the new spool, the employee's clothing got caught in the turning shaft when the left spindle started back up. The employee's left arm was pulled into the machine, resulting in a fractured humerus and lacerations to her triceps. She was hospitalized and required surgery.
