An employee was fixing a lift that was used in a crematory. The lift was propped up with wooden blocks so that the employee could line up the holes for new bolts. One of the wooden blocks moved and the lift collapsed, amputating the employee's right ring finger just above the knuckle.

An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.
