On January 22, 2022, an employee was using a ripsaw to cut a panel. Hearing a noise, he turned off the saw and reached in. The blade amputated his right ring finger at the first knuckle.

An employee opened the door of a clogged filtration system to look for fibers stuck in the system. He was about to tap the side of the fan mechanism to break the debris loose when a fan cut his right ring finger. He suffered an amputation to the finger.

Arauco North America

An employee was working at a bander station. She was asked to help a forklift driver separate a unit of wooden MDF panels which had a bad panel on the 17th panel from the top. The employee was standing on the left side of the forklift, giving directions on fork positioning. The forklift pushed 16 panels from the stack which fell on top of the injured employee's leg. She was hospitalized with two broken bones on her left ankle and torn ligaments that required surgery.

An employee was on fire watch and fell 18 feet from an open catwalk to the ground. The employee was hospitalized for a fractured scapula and a broken rib on the left side, a head laceration and heavy bruising on the right arm.

An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.

An employee was using a table saw to cut a piece of wood for wall framing. The wood jumped, causing the employee's hand to move toward the saw, amputating their left index finger and causing a laceration on their hand.

An employee was removing a jammed piece of scrap wood from the end of a machine saw when the wood pulled their right hand into the saw blade, resulting in an index finger amputation below the second knuckle.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

Big River Steel, LLC

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

American Tile & Marble Co., Inc

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
